NOTICE:
Dear Parents and Guardians,
In order to remain in compliance with the Prairie Lea ISD Medication Administration policy, as well as with Texas State Law, and the Texas Board of Nursing, guidelines for the administration of Over-the-Counter (OTC) and prescription medications have been updated to reflect current school policy. The Texas Nurse Practice Act establishes clear boundaries for nurses explicitly stating that it does not encompass medical diagnoses or prescriptive measures. All medications whether prescription or OTC require a valid order signed and dated by a physician, Nurse Practitioner (NP), or Physician Assistant (PA).
Please review PLISD policy and forms requiring signature(s) for the 2025-2026 school year. Contact the PLISD School Nurse or local Health Department for any questions or concerns.
● PLISD will provide a list of OTC, emergency medications, and treatments that will be on hand in the Nurse’s office.
● If a student requires other medications than those listed, each student will be required to have those medications brought to the nurse in the original containers, with the prescriptions attached if applicable, and a form signed by both the healthcare provider and parent/guardian of each individual student.
● Expired medications will not be accepted and will not be administered by the nurse.
● Parents are required to pick up any unused medication at the end of the school year. Any medications not retrieved by parents will be disposed of by school personnel.
● All PLISD students are required to have these forms completed in full prior to the beginning of the 25-26 school year. *Failure to have this information submitted to the nurse will result in no medications being administered to the student except in an emergency.
● If any of the stocked medications on the list are not to be given to your child, then please indicate this desire by crossing them out on the list.
● Please list any allergies to medications your child may have on the form.
● If your child needs additional medication during the school year, please note that a new form must be completed and returned to the nurse for the new medication to be administered.
● Please make sure all forms have been properly signed and dated.
